How to Use

Dosage

The usual dose for most patients with high blood pressure is one Cozaar Comp 50 mg/12.5 mg tablet once a day.

Method

Swallow the tablet whole with a glass of water, with or without food.

Important

The maximum daily dose is two Cozaar Comp 50 mg/12.5 mg tablets or one Cozaar Comp Forte 100 mg/25 mg tablet.

Possible Side Effects

COMMON
  • Cough, upper respiratory tract infection, stuffy nose, sinus problems.
  • Diarrhea, stomach pain, nausea, indigestion.
  • Muscle pain or cramps, leg pain, back pain.
  • Trouble sleeping (insomnia), headache, dizziness.
  • Weakness, tiredness, chest pain.
  • Increased potassium levels, decreased hemoglobin (a substance in red blood cells).
  • Changes in kidney function, including kidney failure.
  • Low blood sugar.
RARE
  • Anemia (low red blood cells), skin spots, bruises, low white blood cells or platelets, clotting problems.
  • Loss of appetite, increased uric acid (which can cause gout), high blood sugar, unbalanced electrolytes in blood.
  • Anxiety, nervousness, panic attacks, confusion, depression, unusual dreams, sleep issues, drowsiness, memory problems.
  • Tingling or numbness, limb pain, shaking, migraine, fainting.
  • Blurred vision, burning eyes, pink eye, decreased vision, yellow vision.
  • Ringing or buzzing in ears, vertigo (spinning sensation).
  • Low blood pressure (feeling dizzy or weak when standing), chest pain, irregular heartbeat, stroke, heart attack, palpitations.
  • Blood vessel inflammation (vasculitis), often with rash or bruises.
  • Sore throat, shortness of breath, bronchitis, pneumonia, fluid in lungs (causing breathing issues), nosebleeds, runny or stuffy nose.
  • Constipation, severe constipation, gas, stomach upset, cramps, vomiting, dry mouth, salivary gland inflammation, toothache.
  • Jaundice (yellowing of eyes and skin), pancreatitis (inflammation of the pancreas).
  • Hives, itching, skin inflammation, rash, skin redness, sensitivity to light, dry skin, flushing, sweating, hair loss.
  • Pain in arms, shoulders, hips, knees or other joints, joint swelling, stiffness, muscle weakness.
  • Frequent urination at night, kidney problems (including kidney inflammation), urinary tract infection, sugar in urine.
  • Decreased sexual desire, impotence.
  • Swelling of the face, local swelling (edema), fever.
  • Intestinal angioedema: swelling in the gut with symptoms like stomach pain, nausea, vomiting, and diarrhea.
  • Liver inflammation (hepatitis), abnormal liver function tests.
  • Acute respiratory distress syndrome (signs include severe shortness of breath, fever, weakness, and confusion).

Important Warnings

  • Do not take if you are allergic to losartan, hydrochlorothiazide, other sulfonamide medicines, or any other ingredients in this medicine.
  • Do not take if you have severe liver problems.
  • Do not take if your potassium or sodium levels are too low, or calcium levels are too high and treatment has not fixed them.
  • Do not take if you have gout.
  • Do not take if you are more than 3 months pregnant. It's best to avoid this medicine even in early pregnancy as it can harm your baby.

  • Do not take if you have severe kidney problems, your kidneys produce no urine, or you have diabetes and kidney problems and are taking aliskiren.
  • Talk to your doctor immediately if you notice vision changes or eye pain, as this can be a serious condition.
  • If you've had breathing or lung problems after taking hydrochlorothiazide, tell your doctor. Seek immediate medical help if you get severe shortness of breath or difficulty breathing after taking this medicine.
  • Tell your doctor if you have had swelling of your face, lips, tongue, or throat.
  • Tell your doctor if you are taking other blood pressure medicines like ACE inhibitors (e.g., enalapril) or aliskiren, as your doctor may need to adjust your dose.
  • Tell your doctor if you are taking other medicines that can increase potassium levels in your blood.
  • If you have had skin cancer or notice new skin changes during treatment, tell your doctor. This medicine can increase the risk of certain skin cancers. Protect your skin from sun and UV light.
  • Talk to your doctor if you experience stomach pain, nausea, vomiting, or diarrhea after taking this medicine.
  • Avoid drinking grapefruit juice while taking this medicine.
  • Avoid alcohol, as it can make the medicine's effects stronger.

Use in Elderly Patients

Category C

Suitable for older adults, with specific precautions. Effectiveness is reduced in renal impairment. Consider risk of orthostatic hypotension and hyponatraemia and -kalemia. (updated 19.5.2025)
